Categories of Membership:
(the actual membership is held by the practice or company)
For
persons having management responsibilities in a medical or
dental practice or an entity that exist to provide healthcare services to patients.
For
those who offer purchased services and supplies to medical and dental
managers. Associate members must attend 50% of the meetings
and be active on a committee annually. One member per business
firm will be accepted. Associate membership may not exceed
25% of the active membership and membership will be terminated
if requirements are not met. Associate members are eligible
to vote.

MEMBERSHIP IS FORFEITED IF:
• A representative fails to pay annual
dues by January 31 of the year.
• A representative leaves his/her position may
pay prorated dues and continue to be an active individual member for the remainder of the calendar year. The practice
that owned the original membership may send a substitute until a permanent transfer is completed.
